I understand that my participation in any Soultrek, LLC/Laura J. Widman sponsored or initiated session or appointment, workshop, online call/webinar, event, trek, or activity are completely voluntary. I understand that any outdoor/indoor or movement activity may involve risks and hazards or forces of nature not limited to walking and dancing on uneven ground, including actions of other participants or persons. I understand that any interaction, anywhere, with Soultrek, LLC and its members, is not intended to diagnose, treat, prevent or cure any disease or condition. Interactions and sessions are not intended to substitute for the advice, treatment, and/or diagnosis by a qualified licensed professional. I agree to release, hold harmless and indemnify Soultrek, LLC and its members for any and all liability, claims, and causes of action arising out of or in any way connected with my participation or the participation of any minor who I am signing on the behalf of, in any activities offered by Soultrek,LLC. I personally assume all risks in connection with these activities. The terms of this agreement shall serve as a release and indemnity agreement for my heirs, personal representatives and for all members of my family. I understand the conditions above and hereby consent to participate in these activities. By registering/RSVPing for an event or session with Soultrek, LLC or Laura J Widman, I hereby consent to receiving notification emails and/or messages regarding my participation in the event I am registering for and for future events mailing list.
